Washington, 29 December 2025 – The Affordable Care Act (ACA), widely known as Obamacare, is facing a slowdown in health insurance enrolments for 2026, raising concerns over healthcare security across the United States. According to Mehmet Oz, Administrator of the Centers for Medicare and Medicaid Services (CMS), the number of Americans enrolled in ACA coverage is projected to drop to approximately 15.6 million in 2026, down from around 16 million in 2025. Policy experts attribute this decline primarily to the conclusion of pandemic-era subsidies.
During the COVID-19 pandemic, the federal government implemented temporary subsidies to keep health insurance accessible. These enhanced financial supports are set to expire on 31 December 2025. As a result, premiums for subsidised plans are expected to rise sharply in 2026. Government projections indicate that the average annual premium could surge from roughly $888 in 2025 to an estimated $1,904 in 2026, more than doubling. Such increases could place substantial financial strain on lower- and middle-income families.
CMS data also reveal that only 19.9% of current customers have renewed their ACA plans this month, slightly below last year’s 20.5% renewal rate. Mehmet Oz noted on social media that stricter measures to prevent fraudulent or irregular enrolments have contributed to the modest decline in renewals.
A recent survey found that nearly one-quarter of ACA customers—approximately 25%—would consider discontinuing their insurance if premiums double, potentially affecting around 24 million Americans. Enrolment for the upcoming year began in November and will continue until 15 January. Historically, renewal rates rise at the start of January; however, insurers and state officials warn that the absence of additional subsidies may lead to a significant drop this year.
Political uncertainty further complicates the situation. Former President Donald Trump and the Republican Party remain divided over subsidies and alternative healthcare solutions. Despite over a decade of promises to replace ACA, no widely accepted plan has emerged.
The combination of expiring federal subsidies, anticipated premium increases, and political instability has placed the U.S. health insurance system under pressure. The decline in ACA enrolments not only threatens individual coverage but also has wider implications for Americans’ confidence in healthcare security.
